Application:

1. Applicants must provide local address for application and aged 18 or above.

2. For new applicants, collect receipt(s) of minimum HK$2,000 spending with a completed application form, *receipts must be issued on or after 1 st July 2008, return to any restaurant of Iccho Japanese Restaurant or Sushi Nagoyaka Tei or send to Kwun Tong Head Office.

3. For members whom held the membership of year 2006 to 2008, only collect receipt(s) of minimum HK$1,000 spending with a completed application form, *receipts must be issued on or after 1 st July 2008, return to any restaurant of Iccho Japanese Restaurant or Sushi Nagoyaka Tei or send to Kwun Tong Head Office. Successfully applied: the accumulative points will carry forward. No more membership renewal will be accepted from 1 st December 2008 onward, to enjoy exclusive benefit must apply as new applicant.

Member Terms & Conditions:

1. Process of Application: Usually takes 6 to 8 weeks. No instant points accumulation or post entry could be made for the receipts of application and during vertification.

2. Member Card Collection: Should the application have been approved the member will be informed with their Member Card by mail. Should the application have not been approved the applicant will not be informed.

3. Validity Period: Member Card is valid until 30 June 2011.

4. Validity Outlets: Member Card is accepted at any restaurant of Iccho Japanese Restaurant and Sushi Nagoyaka Tei. We can add or reduce the number of restaurants for points accumulation, members can review our website regularly.

5. Privileges Redemption: Member must present Member Card to redeem privileges offers, you may be asked to present identity proof if necessary.

6. Payment Methods: Member Card is only for accumulation bonus points, therefore it is not a credit card or charge card. Cash or credit card shall pay all bill settlement. Member must present the Member Card before bill settlement to accumulate points.

7.  Non-Transferable/ Loss/ Damage: The Membership of point scheme is not transferable, and the bonus points could not combine with other memberships. If the Member Card is lost or damaged, the cardholder shall immediately notify our Promotion Department in writing through fax 2342-5766 or website. For replacement of a lost or damaged card, an administration fee of HK$30 will be charged.

9. Final Decision: Wellmart Management ( Hong Kong ) Limited reserves the final decision to use the Member Card, and amend the Member Card Scheme without prior notice.
Bonus Point Scheme

1. Cardholders who dine in any restaurant Iccho Japanese Restaurant and Sushi Nagoyaka Tei can earn 1 bonus point for every HK$1 spent. Please present the Member Card before bill settlement.

2. 10% service charge is excluded from the bonus point.

3. As accumulating every 3,000 points, members are entitled to redeem cash coupon.

Redemption

As accumulating every 3,000 points, cash coupon could be redeemed at any restaurant of Iccho Japanese Restaurant or Sushi Nagoyaka Tei. Cash coupon is valid for 3 months, which is counted from the redemption day, but the cash coupon could not be used on the redemption day.
